Now is the time to update your child's health records for Kindergarten Registration!

Make plans to complete your child’s health screening and immunizations before the first day of school.
Students who will attend kindergarten in the Fall must submit two state forms as part of the school registration process— the form that shows evidence of vision, hearing, dental, and nutrition screenings in the past year (a current Georgia Form #3300, revised 2013) and a valid Certificate of Immunization (Georgia Form #3231). Your healthcare provider or dentist may complete these forms or you may visit one of the local health department locations to complete the forms.
See the school system website to learn about the two-step registration process for Gwinnett County Public Schools. You should complete Step 1—online registration on the district website—even if your child’s health records are not up to date. In Step 2, schedule an appointment at your child’s new school to verify information, pick up materials, and complete a kindergarten readiness profile.
School health screenings and immunizations are provided at the Gwinnett County health centers located Lawrenceville and Norcross. Check with your preferred location to make an appointment.
