*Certificate of Enrollment and ADAP Certificate requests are placed online via MyPaymentsPlus (Click on "Events and Activities" and then they are listed under "School Fees"). After ordering, you can pick them up from the NGHS front desk the following business day.

Certificate of Enrollment:

The Certificate of Enrollment is a notarized form required to test for both a driver's permit and driver's license. This form is only valid for 30 days. The cost is $5.00 per copy.

Replacement ADAP Certificate:

*IMPORTANT: The school only has access to ADAP certificates for Health courses taken in person, during the school year at North Gwinnett. If a student takes health through Gwinnett Online Campus, Georgia Virtual, or at a different school, they must obtain the form from there. 

ADAP Certificates are only required to test for a driver's license. Students should receive an ADAP certificate upon completion of the Alcohol and Drug Awareness Program when they take their Health course as there is no expiration date for this form, however replacement certificates may be obtained for $5.00 per copy.
